Select what type of request you are making by this form. If you request order for recycling you have agreed to be charged appropriately according to our pricing page.
This can be a work or home contact number for this request. whichever is appropriate.

Describe the items you'd like recycled one per line. Include specifications if you know what they are e.g. 5 Pentium 4 Dell Desktops 128MB RAM, 20GB Hard Drive, DVDROM. Otherwise just itemise in the form of 5 Computers, 3 CRT Monitors, 2 Inkjet Printers etc.
Enter any additional comments or instructions here. For example, "We're leaving the items by the bin out the back of the store, if you need help ask for Ted at the front desk. Please fax us the invoice once you have collected the items."
